Improving Plant Reliability Through Failure Analysis and Motor Condition Monitoring

Deplian Maherdianta, Reliability Manager, PT Pupuk Kalimantan Timur Siis, Staff Reliability Engineer, PT Pupuk Kalimantan Timur

Description

This session will present a case-based discussion on improving plant reliability through structured failure analysis and motor condition monitoring. With insights from PT Pupuk Kalimantan Timur’s fertilizer production facilities, the session highlights how failures in medium-voltage motors were investigated using the 5-Why method, vibration spectrum analysis, and wedge inspection techniques. The implementation of corrective actions based on root cause analysis resulted in reduced unplanned shutdowns and optimized asset performance. The session will also cover the integration of cross-Plant Digital Monitoring (PDM) for proactive failure detection and decision-making.

Learning Takeaways:

  • Understand key techniques in identifying and analyzing motor failures (e.g., winding arcing, wedge degradation, contamination).
  • Learn how structured failure analysis (e.g., 5-Why, RCA) leads to actionable reliability improvements.
  • Explore case-based insights from a large-scale fertilizer plant to apply in similar industrial environments.
  • Gain exposure to digital monitoring strategies (Cross PDM) for early fault detection and prevention.
